TERMS
- To abide by the Constitution and By-laws of the Campbell River TV Association, which are now in force or which from time to time in the future shall come into force.
- To agree to pay all dues, fees and assessments which shall henceforth in the future be lawfully chargeable against you by reason of such membership.
- To agree that this membership and service will not be transferred or made available to any third party as part of a rental or sale agreement.
- To agree to the provisions of By-Law No. 3 of Campbell River TV Association to lend the Society the sum of One Hundred ($100.00) Dollars to be secured by you; by means of a non-interest bearing Debenture.
CONDITIONS (GENERAL)
- Cable accounts are billed, and payable one month in Advance.
- Post dated cheques and cash payments will not be accepted at the CRTV office.
- In case of moving or disconnection, please advise this office 14 days in advance. To terminate service, the subscriber must ensure all rental equipment is returned to Campbell River T.V. Refunds will be made for prepaid service and will be pro-rated to the day of cancellation.
- Service is subject to disconnection without notice on either past due accounts or breach of the service contract. In order to reconnect service, all past due amounts and a reconnection fee must be paid in full and prior to reconnection. All past due amounts are subject to a Late Payment charge.
- Claim of failure to receive, or loss of bill will not be accepted as an excuse for non-payment of account.
- Returned cheques and returned pre-authorized payments will be charged a service fee.
- The ownership of all cable and equipment for the transmission of the television signal up to the termination block, shall be and remain the property of Campbell River T.V. and shall not be removed or tampered with.
- Technicians will rectify any cable problems at no charge, up to and including the termination block.
- Service calls requested for customer owned equipment and wiring beyond the termination block will be done at a service charge, billed to the account.
- Cable TV connection rates are based on the average cost to connect a customer to cable. Customers are responsible for the cost of labour and materials of any additional work, over and above the average time, to connect to cable service.
- Standard signals are provided directly to terminal equipment without rate reduction for subscriber owned equipment.
CONDITIONS (EQUIPMENT)
- Equipment will remain the exclusive property of Campbell River TV Association.
- The equipment must be returned immediately to the Association in good working condition upon cancellation of the Digital Cable or modem service, cancellation of membership in the Association or if your Digital Cable or modem account becomes overdue. Where the equipment is damaged as the result of abnormal use, negligence, tampering or it is lost/stolen: it is acknowledged and agreed to by the member to pay the full replacement value of the equipment. Equipment will be charged to the account if not returned upon cancellation or disconnection for non payment.
- Equipment may not be moved, loaned, rented, assigned or transferred to a third party without the prior written consent of the Association.
TERMS (PAYMENT BY PREAUTHORIZED PAYMENTS)
The Member agrees that:
- Campbell River TV Association is permitted to prepare withdrawals from an authorized bank account or credit card as regular payment for the Association's services.
- The withdrawal may be prepared and communicated by any means in accordance with the regulations prescribed by the Canadian Bankers Association.
- All withdrawals will be credited to the subscriber's account with the Association.
- The amount to be withdrawn is in agreement with the charges specified.
- The date of the withdrawal shall be on the first day of each month or a reasonable time thereafter.
- Association acceptance of payments by preauthorized payments may be terminated at any time without prior notice.
- To notify the association in advance of any request to terminate payments or change an account number.
- Refunds of prepaid charges will be made only to the member.
